A CDAPP Sweet Success Affiliate is a health care provider or site such as a doctor's office or clinic, holding a signed Memorandum of Understanding (MOU) with CDAPP Sweet Success Resource and Training Center. The affiliate staff is trained in the care of women with diabetes, and they follow the standards set in the CDAPP Sweet Success Guidelines for Care.

The MOU defines the agreement between the Affiliate and CDAPP Sweet Success Resource and Training Center and specifies affiliate participation in  training, data collection and completion of an annual affiliate on-line survey.
